По идее тут все предложения должны быть в Present Simple. Но раз в задании просят составить вопрос и отрицание к Past Simple, предлагаю такое решение:

Did you brush your teeth after you got up?

Did you have breakfast at 8 o’clock?
No, I did not have breakfast at 8 o’clock.

7. It usually takes me about fifteen minutes to get there.
